Fatal Incident: A deadly car accident occurred on June 3rd, 2018 on one of the civil works sites. The World Bank team was informed only 8 weeks later on July 27, 2018 by email sent by the coordinator of the PIU to the TTL. Based on the findings of the gendarmerie’s report, the accident resulted from overspeed above the permitted limited on earth road and from a violation of the safety regulations applicable to vehicles transporting workers. The management of the Bank reached out to the ministers of finance and infrastructure to alert them on the seriousness of the accident and the deplorable lateness of reporting the incident ( 8 weeks) to the Bank. Safeguards Incident Response Toolkit (SIRT) was triggered to address the fatal incident. 1/8/2019 Page 1 of 6 The World Bank Implementation Status & Results Report Mali - Rural Mobility and Connectivity Project (P160505) The task team held urgent meetings with the PIU and the line ministry to identify safeguards corrective action plan (SCAP). The civil works were suspended due to the rainy season. The task team and the client agreed to keep all the civil work contracts suspended until the following SCAP was satisfactorily completed: (i) Compensation of the wife of the deceased by the Contractor by End of August 2018;(ii) Establish a timely detailed incident report procedure that will be applied to all works sites by end of August 2018;(iii) Reinforce a continuous mandatory daily briefing of all workers on Occupational Health and Safety, (iv) Equip transport vehicles of both contractors and engineer with speed control devices by end of September 2018; (v) Equip the open cargo bay of the pick-up with chairs and seatbelt for the transport of workers or use minibuses. The PIU implemented all the actions in a satisfactory manner and the task team agreed to resume the civil works on November 1,2018. An Occupational Health and Safety inspection for all contracts will be conducted in February, 2019. The TORs were approved and the recruitment is underway. Rural Roads Improvement Works: There is a delay in the implementation of this component which represents about 80% of the project amount. Whereas the project was expected to complete the rehabilitation of 443 km of roads of phase I by December 31,2018, only about 288 km (65%) have been completed to date. In addition, none of the road contracts were completed within the contractual time. During the monthly progress meeting of December, 2018, the task team, the PIU and the representatives of the beneficiaries concluded that AGETIER ( Agence d’Execution des Travaux et d’Equipement Ruraux) does not have the capacity ( no dedicated qualified personnel) to properly procure and manage the civil works contracts. As a proactive corrective measure going forward, the task team recommends that the PIU handle directly the procurement of the road works of phase II, the National Directorate of Roads handle the technical oversight and contract management of phase II, and the MOU with AGETIER to be terminated. This measure is expected to allow the project to catch up in year 2019 and rehabilitate a total of 800 km of rural roads (phase I and II) before December 2019. Routine Maintenance Works: The NGOs in charge of the formation of the road maintenance workers groups have been recruited. The maintenance groups are expected to be operational after the next rainy season and to perform routine maintenance works on the roads of phase I. Social Audit: The social audit concluded that some contractors did not respect the legal Right of Way of the construction sites. In consequence few trees belonging to some farmers were affected. The losses were estimated in the audit report, and the Contractors were notify to compensate the farmers before end of january, 2019. Community Monitoring: The first campaign of the community monitoring was globally satisfactory. Focal points equipped with smartphone and questionnaire were able to send beneficiary feedbacks about the civil works of phase II. The feedbacks were positive on road sections were the physical progress was satisfactory and negative on road sections with slow physical progress. In some locations, the beneficiaries were not satisfied with the temporary runaround during raison season. The PIU conducted a verification mission and later inform the contractors to properly construct the runaround so that villagers can have basic access during rainy season.
